Case Management & Social Services
Curry Health Network provides Case Management and Social Services for our inpatient population at Curry General Hospital.
Case Management consists of a Licensed Clinical Social Worker (LCSW) who works closely with the insurance providers of our patients, and coordinates hospital discharges. Our LCSW also works closely with providers; nurses; physical, occupational and speech therapies; and other healthcare professionals to assist patients in the hospital. They are part of the care team and participate in daily rounds with the medical staff.
They are often called upon to resolve clinical and psychosocial concerns and to identify and remove barriers to enable a patient's timely discharge from the hospital. The LCSW is also available to discuss and educate patients and families on placement options into various types of care facilities, coordinate care with home health, hospice, and state agencies, assist and educate in advanced care planning and provide general resource assistance to patients and their families.
Our LCSW is available to provide social services Network-wide, as needed, and to the community at-large as a point of contact for resource assistance. Our LCSW also works with Curry Health Network's providers at our clinic locations in Gold Beach (Curry Medical Practice), Port Orford (Curry Family Medical) and Brookings (Curry Medical Center), to coordinate care for our patients.
Social Services include, but are not limited to:
Facilitating communication and coordination of care between Curry Health Network, other community partners, patients and families of our patients
Providing supportive counseling to patients and their families
Assisting patients facing addiction and chronic pain
Assisting those coping with terminal illness, death, disability, and social and mental health problems
Our LCSW is trained to treat behavioral and mental health issues as well as counseling and therapeutic techniques, allowing them to offer clinical and mental health services when needed.
Our LCSW acts as an advocate on behalf of our patients, which may include making the patient's wishes known to their family and treatment team, or ensuring the patient receives accurate information about his/her care and treatment. The LCSW also assists patients with advanced care planning, including advanced directives, which offer clear instructions as to the wishes of patients related to medical treatment options and end-of-life decision-making.
